About Team Select Home Care

Team Select Home Care serves medically complex children and adults across Porter, Lake, LaPorte, Jasper, and surrounding Northwest Indiana counties, with our office in Valparaiso. Patients & Families: We offer pediatric and adult PDN, including ventilator care, trach management, and 24/7 skilled nursing, plus Personal Attendant Services and Indiana's Family Caregiver Program — where eligible family members can be paid to provide care at home. Referral Sources: We coordinate with physicians, Indiana Medicaid, and insurance for authorizations and complex case management. Nurses & Caregivers: RN, LPN, and caregiver openings with flexible schedules, new grad training, and tuition reimbursement.
